IMM 5484 (01-2018) E – Document Checklist for Temporary Resident Visa

Important Note:

• This checklist is for reference only. Refer to the Instruction Guide (IMM 5256) for complete details.
• Ensure all documents are in English or French. If not, translation might be required.

Gather your documents as per the checklist and check (✓) each item.

Required Forms (Complete, sign and date):

• ✓ Application for Temporary Resident Visa (IMM 5257) – Must be validated for barcode generation if completed electronically.
• ✓ Family Information Form (IMM 5707) – Specific to your application region.
• ✓ Statutory Declaration of Common-law Union (IMM 5409) – Check with your regional visa office.
• ✓ Use of a Representative (IMM 5476) – Only if using a representative.
• ✓ Authority to Release Personal Information (IMM 5475) – Only if authorizing release of your case file information.

Two Photos:

• ✓ Meet visa application photograph specifications.
• ✓ Write your name and date of birth on the back.
• ✓ Not required if biometric fingerprints and photo are mandatory.

Proof of Financial Support:

• ✓ Documents showing you can financially support yourself and accompanying family members during your stay.

Additional Documents:

• ✓ Marriage License/Certificate photocopy (Check with your regional visa office).
• ✓ Purpose of Travel document (Check with your regional visa office).
• ✓ Current immigration status photocopy (if your country of residence differs from your passport’s citizenship).
• ✓ Custody documents or authorization letter (for minors traveling alone or with one parent).
• ✓ Any additional documents required by your regional visa office.

Document List:

• ✓ Fee payment (application processing and biometric fee, if applicable) using an acceptable format (check with visa office/VAC).
• ✓ Photocopy of your valid passport or travel document information page (including passport number, issuance/expiry dates, photo, name, date and place of birth).
  • Ensure one completely blank page (other than the last page) is available in the passport (check with your regional visa office).

Parents and Grandparents Super Visa (Additional Documents):

• ✓ Evidence of parent or grandparent relationship to the Canadian citizen/permanent resident (birth certificate, baptismal certificate etc.).

• ✓ Proof of private medical insurance coverage for a minimum of one year with a Canadian insurance company (copy of the certificate or policy).

• ✓ Letter of invitation from your child or grandchild in Canada (permanent resident or citizen).

  • Include host’s family composition (dependants).

• ✓ Proof that your child or grandchild meets the Low Income Cut-Off (LICO) minimum (one of the following):

  • Most recent Notice of Assessment or T4/T1 tax document.
  • Original employer letter stating job title, description and salary.
  • Employment insurance pay stubs.
  • Letter from accountant confirming annual income (if self-employed).
  • Proof of other income sources (pension statement, investments etc.).
